By reviewing activity, outcomes, and service use, teams can make consistent, evidence-based decisions and ensure resources are focused on those with the greatest clinical need.
Self Management Model
We have developed a well-established Self-Management model designed to embed self-management safely and effectively within organisations, ensuring patient safety remains paramount at all times. The model also provides clear guidance for clinicians to identify individuals who are appropriate for this approach. It was originally developed by Stephanie, Director of SMC, during her time at Leeds Community Healthcare, where she led the creation of the first dedicated self-management team in the country. This work demonstrated significant impact, saving approximately 3,000 community visits per month. The model was subsequently implemented within acute trusts across Leeds, where it continued to show positive outcomes, preventing around 73 individuals per month from requiring ongoing community visits.
Well being days and CBT sessions
We are pleased to offer CBT-informed sessions and wellbeing days designed to promote emotional health, build effective coping strategies, and support overall wellbeing.
Self Management Consulting runs a weekly CBT clinic at the Yorkshire Wellness Clinic, providing support across a wide range of needs using practical, evidence-based approaches.
We also deliver CBT sessions for NHS organisations, particularly during periods of change, helping teams to build resilience, manage stress, and maintain wellbeing in the workplace.
